  1. Open a New Document:
    • Start Microsoft Word on your computer.

  2. Design the Feedback Form:

**a. Header:

    • Insert a Header:
      • Go to the Insert tab and select Header.
      • Add a title like "Employee Evaluation Feedback Form".
      • Include your company’s name and logo if applicable.

**b. Employee Information:
